New Patient Information
At Restore Behavioral Health, it our desire to make the New Patient Registration process as streamlined as possible. Our highly trained staff are here and eager to help make this a pleasant experience. Below is outlined some of the information that you may need to facilitate a smooth New Patient Registration process.
Clinic Hours: Monday through Friday, 9am to 8pm. Saturday 10:00am – 4:00pm
To schedule, change, or cancel an appointment:

Cancellation Policy: Any patient that cancels an appointment with less than 24 hours notice and/or does not show up for their appointment will be charged a fee of:
- $45 for Visits on Monday through Friday
- $45 for Visits on Saturdays
- $75 for Testing Visits on any day
Clinic voicemail and email are available for cancellations after hours
The initial appointment is a Diagnostic Assessment Intake. During this appointment, a Clinical Interview will be conducted during which the patient’s symptoms, history, background information, concerns, and initial treatment planning are reviewed
At this time a therapist will be assigned and a plan for any additional psychological/neuropsychological testing or QEEG Brain Mapping will be determined.
RBH offers a staff of highly trained Psychologists, Social Workers, and Licensed Professional Counselors and Associates to assist in the patient’s testing and therapy.
RBH will also coordinate with any referring physicians at this time if requested by the patient/family. Additional outgoing referrals may be provided as well at the end of the intake appointment.

Fees and Payments
All Copay, Coinsurance, or Fee for Service payments are due at time of service delivery.
If you are unsure of the cost of your appointment, our friendly administrative team can provide this information to you. Our office will contact your insurance company as a courtesy to you to determine what your plan coverage details for copay/coinsurance payments.

Are you accepting New Patient Referrals?
Do you prescribe medication?
No, Restore Behavioral Health does not have a Psychiatrist on staff. If treatment warrants medication management, an psychiatric medication consult referral will be provided to the patient.
Do you specialize in Court-Related Cases?
No, RBH services are designed for diagnostic and treatment purposes only. Referrals can be provided if you are in need of any Court-Related Psychological Services.

Will anyone know that I came there for therapy?
No, not unless you specifically provide a written Consent to Release of Information. Our staff is trained to follow all HIPPA guidelines and state laws regarding the protection of patient confidentiality.
