Below is information to assist you with a smooth New Patient Registration process.
Appointments

Cancellation Policy:
Any appointment canceled with less than 72 hours of notice, including no-show at the time of service delivery, will incur the following fees:
- $45 for Therapy, Test Results, and Neurofeedback
- $75 for Testing and QEEG Brain Maps
First Appointment:
The first appointment is a Diagnostic Assessment Intake. During this appointment, a Clinical Interview will be conducted during which the patient’s symptoms, history, background information, concerns, and initial treatment planning are reviewed.
At the intake appointment, a therapist will be assigned and a plan for any psychological/neuropsychological testing or QEEG Brain Mapping will be determined. The RBH clinical team includes highly trained Psychologists, Licensed Professional Counselors, and LPC Associates.
RBH will coordinate with referring physicians if requested by the patient/family. Additional outgoing referrals may be provided at the end of the intake appointment.

Insurance
Restore Behavioral Health accepts many insurance plans:- Aetna

Please note that we do not accept HMO plans or Medicaid.
Fees and Payments
Payment: Payments are due at the time of service delivery. When using insurance for your appointments, our office will contact your insurance company as a courtesy to you. Your cost for services will be based on benefit information provided by your insurance company.Payment Methods Accepted:

Are you accepting New Patient Referrals?
Yes.
Do you prescribe medication?
No, medication is not prescribed at Restore Behavioral Health. If treatment warrants medication management, a psychiatric referral will be provided.
Do you accept Court-Related Cases?
No, RBH services are for diagnostic and treatment purposes only. Referrals can be provided if you need Court-Related Psychological Services.

Will anyone know that I came here for therapy?
No, not unless you specifically provide a written Consent to Release of Information. Our staff follows HIPAA guidelines and state laws regarding the protection of patient confidentiality.
